Public Health Event and Syndromic Surveillance

This form is an electronic copy for weekly public health event and syndromic surveillance reporting. It is a substitute to the PHESS booklet that were routinely used. This form will be utilized by all health facilities in West Sepik Province to maintain weekly surveillance reporting of urgent events and outbreak prone illnesses and other health threats. Early detection of diseases from this reporting will inform swift response to prevent, reduce and minimized implications on our people's health.
All health facilities are encouraged to report on a weekly basis using this form. Your data will be stored at the Provincial level and disseminated for actions at various facility level.

*Ensure that the person filling this form is on site at the facility
*Make sure all sections are filled accordingly
*Reporting dates, week and time are based on our weekly schedule for reporting.
