MATERNAL AND NEONATAL MONITORING TOOL

The Hospital-Based Maternal and Neonatal Monitoring Tool (H-MNT) is a weekly surveillance and reporting tool designed to strengthen maternal and newborn health monitoring across health facilities. The tool systematically captures key indicators on maternal deliveries, obstetric complications, neonatal admissions, neonatal complications, mortality outcomes, referral status, and healthcare provider involvement in newborn care. By providing timely and standardized data collection, the H-MNT supports early identification of trends, facilitates quality improvement initiatives, enhances accountability, and informs evidence-based decision-making for reducing maternal and neonatal morbidity and mortality. The tool is adaptable for use at dispensary, health centre, district, regional, and zonal hospital levels.
