Suggested format for a national animal disease information system
Two linked forms for passive disease surveillance.
First (Reporter) form to be filled in by the person notifying about the animal disease event.
Second (Vet/ Lab) form is to record subsequent details of investigation of the case, through to a final veterinary diagnosis. It lists the national priority diseases and WOAH listed diseases diagnosed (as well as negative tests for WOAH listed diseases for potential proof of freedom claims).
